1. Acceptance of Terms

By paying for and utilizing the services provided by LabLink Advanced Mobile Phlebotomy and Courier Services (“LabLink,” “we,” “us,” or “our”), including in-office and mobile phlebotomy, vitamin shots, direct-to-consumer lab testing, and drug testing, you (“Client,” “you,” or “your”) agree to be bound by these Terms of Service. If you do not agree with any part of these terms, you must not use our services.

2. Services Provided  

LabLink provides the following services:

  • In-office and mobile phlebotomy services.
  • Administration of vitamin shots.
  • Direct-to-consumer lab testing services.
  • Drug testing services.
  • Courier services related to lab samples.

3. Client Responsibilities

  • You agree to provide accurate and complete information necessary for the provision of our services.  
  • You agree to follow all instructions provided by our staff.
  • You are responsible for obtaining any necessary consents or authorizations for testing, including drug testing.
  • You are responsible for payment of all fees associated with our services.
  • You agree to provide a safe and sanitary environment for any mobile phlebotomy services.

4. Payment and Fees

  • Payment is due at the time of service or as otherwise agreed upon.
  • We accept debit/credit cards, Apple Pay, and Google Pay.
  • Fees for our services are as published on our website or provided in a separate agreement.
  • Late payments may be subject to additional charges.
  • Payment for any of our services serves as an automatic binding agreement to these terms of service.

Please be aware that all fees are non-refundable. Should services be unable to proceed due to reasons attributable to the client, a $35 rescheduling fee will apply. LabLink reserves the right to decline rescheduling requests for reasons attributable to the client. Additionally, cancellations made within 72 hours of the scheduled appointment will result in the forfeiture of all fees. By processing payment, you acknowledge and agree to these terms and conditions.

5. Cancellation and Rescheduling

  • Cancellation or rescheduling of appointments must be made [Number] hours in advance.
  • Failure to provide adequate notice may result in a cancellation fee.
  • Any fees associated with specimen viability due to client errors, such as missed appointments, will be the sole responsibility of the client.

6. Confidentiality and Privacy

  • We adhere to HIPAA regulations and maintain the confidentiality of your health information.
  • We will not disclose your personal information to third parties except as required by law or with your consent.

7. Test Results and Accuracy

  • We will make reasonable efforts to deliver accurate test results.
  • However, we do not guarantee the accuracy of test results, as they may be affected by various factors.
  • Test results are provided for informational purposes only and should not be used as a substitute for professional medical advice.
  • We utilize partner labs for testing, and are therefore reliant on their data.

8. Drug Testing Specific Terms

  • You agree to comply with all applicable regulations regarding drug testing.
  • We are not responsible for the consequences of positive drug test results.
  • Chain of custody procedures will be strictly followed.
  • Results will be released to designated parties per client instructions and legal requirements.

9. Limitation of Liability

  • LabLink shall not be liable for any indirect, incidental, consequential, or punitive damages arising from the use of our services.
  • Our liability shall be limited to the amount paid by you for the services.  
  • LabLink is not responsible for any adverse reactions to vitamin injections.

10. Indemnification

  • You agree to indemnify and hold LabLink harmless from any claims, damages, or expenses arising from your use of our services or your breach of these Terms of Service.

11. Governing Law

  • These Terms of Service shall be governed by and construed in accordance with the laws of the State of Georgia.  

12. Changes to Terms of Service

  • We reserve the right to modify these Terms of Service at any time.
  • Changes will be posted on our website, and your continued use of our services constitutes acceptance of the changes.

14. Severability

If any provision of these Terms is held to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable.
